Terms and Conditions of Service Correct from Jan 2025

1. Physiotherapy Provision
2. All assessment and treatment sessions will be carried out by an HCPC registered physiotherapist and tailored to the individual, based on contemporary evidence-based practice using a goal related approach. 
3. Each client will have individual arrangements made for each. Intervention will be conducted at a client’s property or in an agreed community location unless indicated.
4. Intervention sessions will last a minimum of 45 minutes. Maximum intervention time is dependent on the type of session carried out and will be discussed with the client on an individual basis. Typical length of sessions will be agreed following the initial assessment.
5. Neuro Herts Rehabilitation LLP will maintain client confidentiality at all times and adhere to all aspects of the General Data Protection Regulation when handling client related information. Details of our GDPR Compliancy is available on request.
6. Following assessment and during follow-up appointments we will discuss treatment options including their benefits, risk and any potential side effects. You will maintain the right to decline treatment at any point.
7. Your consent will be gained prior to any sharing clinical information or reports with any professionals involved with your care. This may include information to guide care plans or onwards referrals.
8. Fees and Invoicing
9. The funding party or client confirms that they will fund the intervention outlined for the individual and that there are sufficient funds available to pay for the agreed intervention. Intervention and billable expenses are as follows:
10. Physiotherapy Assessment and Treatment Fees: 
    1. Initial Assessment: £130. Up to 90 minutes allowed.
    2. Follow-up: £100. Up to 60 minutes allowed. 
11. Physiotherapy assessment and treatment may include Face to face intervention and assessments with a client either in their home or in community setting, completion of reports, relevant client related research and planning intervention, training to support workers and relevant others, joint assessments with other team members or equipment reps, meetings with family, clients, clinicians and relevant others, client related telephone calls. 
12. Report Writing:

Comprehensive reports are available on request and charged at £100 per hour. Completion of Initial Assessment reports will be charged a maximum of four (4) hours work. Invoiced as ‘Initial Reports’. Completion of Progress Reports will be charged at £90 per hour for a maximum of three (3) hours work. Invoiced as ‘Progress Reports’.

For clients who are part of a litigation process an additional 15 minutes is charged for completion of treatment notes relating to each session. 

1. Travel Costs and Time

Appointment locations outside a 15 minute travel radius from SG5 1SQ may incur additional travel and expense charges. These will be costed and agreed prior to the first contact. 

1. Accounting of Time: 

All fees shown on invoicing will be actual time and monetary equivalent. Invoices will be itemised to the type and amount of intervention provided for the client. 

1. Review of Fees: 

Neuro Herts Rehabilitation LLP reserves the right to increase fees with reasonable notice. A minimum of one calendar month will be given of any increase in fees.

1. Invoice payments: 

If payment is not made at the time of assessment and/or treatment, then an invoice will be generated and payments must be made within 7 days. If payment is not received Neuro Herts Rehabilitation LLP reserves the right to charge a £40.00 late fee for each outstanding invoice. Payment via bank transfer is preferred method. Account details are on each invoice produced. Cheques are not accepted.  Supply of professional services by a registered physiotherapist is VAT exempt.  

5. Cancellation of appointments: 

1. If planned appointments or meeting cannot go ahead, they should be cancelled or rearranged with a minimum of 24 hours’ notice by the client, case manager or representative acting on their behalf. 
2. In the case of cancelled appointments with less than 24 hours’ notice, Neuro Herts Rehabilitation LLP reserves the right to charge for a minimum of one-hour intervention and up to a maximum of three hours intervention, dependent on the intended session. Travel will be charged at the scheduled rate if travel to the cancelled appointment has taken place. Neuro Herts Rehabilitation LLP will also charge any prepaid and non-refundable expenses incurred.

6. Terminating Intervention

1. Neuro Herts Rehabilitation reserves the right to suspend or terminate intervention to a client in the event of ongoing and persistent non-payment of fees or if the safety of an individual is felt to be at significant risk. In the unlikely event of the above, every effort will be made to resolve the issues. If this is not possible then a written notice of termination will be provided to the client, advocate, case manager or funders. 
2. Clients, advocates or funders may terminate intervention from Neuro Herts Rehabilitation LLP at any time with written notification.
